Person-Centered Care

We prioritize person-centered care, ensuring that everyone’s unique needs and preferences are at the forefront of our services. This approach is embedded in all our programs, from integrated health services to in-home prenatal support. Our team works collaboratively with individuals and families to develop personalized care plans that promote long-term well-being and empowerment.

Comprehensive Support

We provide comprehensive support that addresses the multifaceted needs of our clients. This includes medical care, mental health services, substance use treatment, and family support. Our integrated health services division, care management teams, community health workers and treatment staff work together to offer seamless and continuous care.

Community Empowerment

We believe in empowering communities by providing the resources and support needed to foster healthy, thriving environments. Our programs are designed to strengthen individuals and families, promoting overall community well-being. Through initiatives like our Community Consortium and community outreach efforts, we equip families with the knowledge and skills to create nurturing and supportive home environments.

Collaborative Care Coordination

Effective care coordination is key to ensuring that our clients receive the best possible support. We focus on creating a network of care that includes medical personnel, case managers, and drug treatment counselors and community health workers. Our teams work closely with clients to navigate their health journeys, providing guidance and resources every step of the way.

YOUR SUPPORT TEAM

Clinical Team

Our clinical team is lead by a qualified medical director and includes highly trained Nurse Practitioners and Licensed Clinical Social Workers who provide medical and mental health care. They are dedicated to diagnosing, treating, and managing a wide range of health conditions, ensuring comprehensive and compassionate care for all individuals.

Certified AOD Counselors

Our Certified AOD Counselors will assist you in recognizing the help you need with alcohol or drugs, motivate you throughout the counseling process, and provide counseling services that help you develop and/or maintain a responsible and functional recovering lifestyle. Recognizing the limits of knowledge, skills, and experience, our AOD certified counselors can also refer to other appropriate professional services as needed.

Care Managers

Our Care Managers are responsible for coordinating and overseeing the care plans for our clients. They work closely with individuals and families to ensure they receive the necessary services and support, helping them navigate the complexities of the healthcare system.


Community Health Workers

Our Community Health Workers serve as vital links between the community and healthcare providers. They offer education, support, and resources to help individuals and families maintain healthy lifestyles and access essential services.

Family and Service Coordinators

Our Parent and Service Coordinators are dedicated to connecting families with the resources and services they need. They work to ensure that parents and caregivers have access to the support necessary for their children’s growth and well-being.

Parent Educators and Home Visitors

Our Parent Educators and Home Visitors provide personalized support to parents and caregivers. They offer guidance, practical tips, and evidence-based strategies to foster child development and create nurturing home environments.
